Yale School of Nursing Center for Excellence in Chronic Illness Care Award for Excellence in Caring in Chronic Illness


Purpose

The award acknowledges consistent excellence in promoting or delivering care for people with chronic illness and their families.

Eligibility Criteria

The award may be presented annually to an individual, a group or an organization. More than one award may be presented annually but if so, at least one awardee must be a nurse. The awardee(s) must have a connection with Yale School of Nursing or a Yale School of Nursing clinical site. The award may be received only once although candidates may be nominated more than once. The fields from which nominees may come include, but are not limited to nursing, social work, medicine, administration, clergy, hospice, psychology, and patient advocacy. Nominations may be made by colleagues, patients, and/or families.

Criteria for Selection

  1. The nominee has demonstrated a sustained record of excellence in the care of persons with chronic illness and their families, including:
  2. Serves as an example and role model to others
  3. Creates innovative approaches to the provision of compassionate care
  4. Promotes quality of life for persons with chronic illness and their families
  5. The nominee has a defined connection with Yale School of Nursing.

Nomination Process

  1. The application must be completed by the nominator. The application must by typewritten or printed.
  2. The application must be submitted to the Excellence in Caring Award Committee by December 10, 2004.
  3. The application is strengthened by a resume or biosketch of the nominee.
  4. The application must include the following
    Name of nominee (with credentials)
    Address of nominee
    Telephone numbers of nominee, day and evening
    Nominee's current position (title, employer, and brief description of position and responsibilities.
    Name of nominator
    Address of nominator
    Fax number of nominator
    Telephone numbers of nominator, day and evening
    E-mail address of nominator
    How long have you known the nominee?
  5. Using no more than a total of 2 pages for the application, address each of the Criteria for Selection and describe how the nominee meets each of the criteria.
  6. The nominations will be reviewed by the Excellence in Caring Award Committee. The committee will select the awardee.
  7. Completed applications will be acknowledged by mail or E-mail.
  8. The Award Committee will consider only complete applications.
  9. The nomination process is confidential.

Presentation of Award

The Award for Excellence in Caring in Chronic Illness will be presented during the Center for Excellence in Chronic Illness Care February Annual Convocation.
