Mission of the Center
The Mission of the Center on Self and Family Management for Vulnerable Populations is to promote the creation of knowledge that will lead to the understanding, development and testing of interventions to support self and family management of health problems in vulnerable populations.
Cores and Descriptions
The Center promotes synergy among investigators and interdisciplinary teams through collaboration, the use of shared resources, increasing communication, developing young scholars, and disseminating findings to the clinical, research, and policy communities, and the public.
The Center's framework (.pdf) provides a collaborative, interdisciplinary focus on self-management that aims to improve health and quality of life in people of all ages and in all stages of their health continuum. It is organized into the following five cores:
Administrative Core
The purpose of the Administrative Core is to provide oversight of the Center and to facilitate the achievement of the scientific and educational goals of the other cores. The Administrative Core coordinates the functions of the other cores to ensure optimal use of shared resources.
The core makes use of the combined resources of the External Advisory Board and the Community Advisory Panel to counsel researchers and practitioners, and to advocate the illness management needs of vulnerable populations.

Vulnerable Populations Core
The goal of this Core is to maintain the Center's focus on self and family management for vulnerable populations, to promote scientific interchange at the highest level, and to establish an environment where investigators can interact with and learn from each others' work.
The core works closely with the Josiah Macy's Yale-Howard Interdisciplinary Health Scholars Program to ensure a higher degree of confidence in our data collection and improved accuracy in both the interpretation and dissemination of study findings.

Methodologic Support Core
The Methodologic Support Core has two major functions:
- The direct support of pilot/feasibility studies funded through the center
- The development of faculty's ability to undertake cutting-edge research related to self and family management of health problems in vulnerable populations
Direct research support services are provided for all stages of the research process including consultation on research design, linkage to University resources, support for data processing and analysis, and editorial and presentation development services.
This Core works in tandem with the Dissemination Core to support manuscript development through substantive and editorial reviews.

Outcomes Core
The purpose of the Outcomes Core is to facilitate the measurement of outcomes and to develop new approaches for assessing outcomes. There are two major functions:
- Assessment of outcomes across populations
- Development and assessment of appropriate measures
The Outcomes Core emphasizes the identification and measurement of common outcomes of self and family management interventions across studies. Consistent with national trends, the Center focuses on priority health problems, especially within vulnerable populations, and the creation of a comprehensive measurement set applied both within and across the individual studies.

Dissemination Core
The Dissemination Core focuses on communicating and translating to key stakeholders in practice and policy arenas the Center's research findings related to individual and family self-management. The major purposes of the Dissemination Core are to support evidence-based practice and apply data to organizational and public policy decision-making. The functions of the dissemination core are to:
- Identify the areas of content related to self and family management in vulnerable populations to be disseminated
- Identify target audiences and stakeholders for dissemination
- Develop strategies for dissemination based on the scientific literature and format for disseminating
- Develop and sustain relationships with selected stakeholders (individuals and organizations) related to vulnerable populations
- Develop an evaluation plan to measure our impact
