Modelling HIV Subjects' Electronic Monitoring Device Data. Specific AIMS are 1. Identify subperiods within the EMD observation period over which a subject or a group of subjects exhibits distinctly different adherence patterns, 2. Identify the dependence on time and the variability in adherence for a subject or a group of subjects, 3. Identify classes of subjects with distinctly different adherence across those classes and similar adherence within those classes for evaluation of possible differential effects of an intervention across those classes.
Fennie (PI) Yale-Howard Partnership on Reducing Health Disparities Pilot Program. Health care needs of the transgendered. Specific AIMS are: (1)To describe, via interviews, transgender individuals' thoughts, feelings, and experiences of health care, and their level of trust in their healthcare provider; (2)To describe, via interviews, female to male transgendered individual's thoughts, feelings, and experiences of gynaecologic healthcare, and male to female transgendered individual's thoughts, feelings, and experiences of urologic healthcare. (3) To describe, via interviews, health care providers' (who have experience caring for transgender patients) thoughts and ideas on issues of access, trust, and need in the context of the transgender patient; (4)To quantify, through instruments, level of trust between patient and physician, and level of social support; (5) To determine the general interest and acceptance among transgender of a proactive patient-led approach to educating their health care providers.
Fennie (PI) Renfield Foundation. Translation of a Hospice in Prisons Model to a VA Setting. The specific aims are as follows: (1) To collect data on a sample of veterans regarding their thoughts and feelings on death and dying, their feelings on religion and spirituality, and their family and social support networks which will be compared to data collected from the Hospice in Prisons program. (2) To determine the general interest and acceptance among staff and patients of a peer driven hospice care model within Rocky Hill VA; (3) To translate the curriculum for training and the role activities in care giving from the prison model into a VA model.
Pettigrew (PI) Molecular epidemiologic identification of pneumococcal tissue specific genes.
The specific AIMS are: (1) To determine the molecular epidemiology of S. pneumoniae clinical and carriage isolates from children <5 years of age using MLST. (2) To identify tissue specific virulence factors associated with S. pneumoniae strains that cause clinical diseases in children <5 years of age and evaluate their distribution among a large collection of clinical and carriage strains. (3) To evaluate otitis media associated virulence genes (identified in aim 2) in the chinchilla model of otitis media.

The poor person does not exist as an inescapable fact of destiny. His or her existence is not politically neutral, and it is not ethically innocent. The poor are a by-product of the system in which we live and for which we are responsible.
As health care workers, we must recognize not just that poor are at increased risk for disease and premature death, but that we must focus on serving the poor and be aware of the asymmetry and the inter-dependence of power and poverty.
