2020 New Student Checklist

The Office of Student Life and Belonging is available to assist you in getting ready for your studies at YSN and your upcoming move to the New Haven area. On this page are items that may help you prepare for your arrival in the fall. Of course, if you have specific questions, please don’t hesitate to contact us. 

Immunization & Physical Requirements

All new students are required to submit immunization and physical exam forms. We realize that access to clinics and your PCP will vary depending on your location and services impacted by the pandemic.  Students should first check with their PCP to see how much of the health requirements can be completed by their PCP’s office. Click here to download the forms. Yale University ID Card

Yale University requires that all students be issued an official Yale photo identification card. You will need this card to gain access to  University buildings, libraries, recreational facilities and other services.  Upload your  ID photo by May 31, 2020.

BLS (CPR) Certification

As a condition of enrollment, all students are required to have a Basic Life Support (BLS) certificate for the Healthcare Professional through the American Heart Association. If you are already certified, please be sure your certification is valid through December 2020. To find out more information about the American Heart Association’s BLS training, visit their website or call their toll free number 1-800-242-8721. Course availability will vary depending on sites impacted by the pandemic. Students must upload a copy of their BLS (CPR) certification to E*VALUE by August 1, 2020.


The Health Insurance Portability and Accountability Act (HIPAA), establishes security standards and privacy standards for the use and disclosure of individually identifiable health information. HIPAA applies to health care providers and health plans. At the University, the Schools of Medicine and Nursing, the Department of Psychology Clinics and Yale Health are covered by the regulations and all faculty/staff/students affiliated with these areas must complete the required HIPAA training, and may be held individually responsible and liable if there is misuse of PHI. Please complete your Foundational HIPAA Privacy and Security Training by August 1, 2020.

Mandatory Background Check

The YSN Procedure for Criminal Offense Record Investigation (CORI) of Students. You must complete the background check by May 31, 2020.

Financial Aid Paperwork

Financial Aid paperwork for Federal Loans is due June 1, 2020.

Seasonal Influenza Vaccination (Post-Enrollment Requirement)

These requirements are to be met after starting the program in August (more information will be sent to you):

  • Completion of annual seasonal influenza vaccination – unless medically contraindicated.
  • Education in blood-borne/universal precautions and airborne pathogens as outlined in the OSHA Standards.

Program Specific

Incoming RN/PMC: All incoming RNs are required to have a valid CT RN License. Information on how to submit your license will be sent to your Yale e-mail addresses. Please upload this certification NO LATER THAN August 1, 2020

Orientation Schedules

Orientation for all new students will be Monday, August 24 through Friday, August 28.
