April Mercadante Feld

April is currently the patient service manager for the Assessment Services at The Visiting Nurse Service of New York (VNSNY). In this role, April is entrenched in population health and oversees a dispersed team who perform a variety of comprehensive assessments so that an individualized care plan can be developed accordingly. In December 2012, April along with other colleagues at VNSNY, including the CEO, was honored for services rendered in New York City during Superstorm Sandy. During the disaster, April helped organize and implement a number of initiatives focusing on relief efforts that helped enable residents to re-connect with their community resources to restore life at home. April has worked with VNSNY since 2007, where she began as an intern moving up to patient services manager in 2012. She also serves as an adjunct professor in community health at New York University College of Nursing (NYUCN), where she continues to promote community nursing practice and its profound impact on patient/population health management. April received both her bachelor’s and master’s degrees from NYUCN.
