Job Title: Nurse Practitioner, Psychiatric-Mental Health

Reports To: Comprehensive Psychiatric Program Manager

Job Summary: Provides and manages a high standard of behavioral health medication management and care for patients.                                                                                                                                              

Essential Job Duties and Responsibilities:

The job duties and responsibilities listed below are representative of the knowledge, skill, and/or ability required.  Other duties that fall within the broad scope of this classification may be assigned.

  • Provides and manages direct care to children, adolescents, and adults (depending on licensing/training), including the comprehensive psychiatric diagnostic evaluation, follow-up evaluations, basic medical assessments, diagnostic evaluations of labs and imaging or other diagnostic procedures, and treatment for patients within scope of practice.
  • Diagnoses behavioral health and substance use disorders per appropriate DSM-5 and ICD-10 standards.
  • Prescribes psychopharmaceuticals and treatment regimens as appropriate. Refers patients to primary medical provider and other specialists as needed and relevant to patient care as appropriate.
  • Collaborates and communicates with other disciplines, specialties, and agencies across the healthcare continuum.
  • Directs formulation and implementation of multidisciplinary treatment planning that incorporates the patient’s view, values, and beliefs, and that promotes recovery as defined by the patient.
  • Directs and coordinates the patient care activities of nursing, care coordinators, and support staff.
  • Maintains appropriate boundaries while providing a therapeutic relationship, communication, and counseling/psychotherapy.
  • Provides ethical, culturally competent, trauma-sensitive, recovery-oriented, evidence-based care.
  • Identifies patients at risk to self or others and takes steps to ensure the patients’ safety and the safety of others.
  • Ensures completion of tasks, patient labs, imaging, and reports, as well as required paperwork within designated time frames established in policy.
  • Maintains patient records and provides accurate billing data.
  • Participates in specified health promotion, education and/or prevention programs as appropriate.
  • Participates in providing regular and meaningful peer review and feedback to other medical providers.
  • Assists in facilitating and providing training and mentorship to improve and advance professional skills and competence of medical support and other clinical staff.
  • Participates in education of clinical students, as well as community partners and undergraduate students, per the guidelines of CCHCI medical education programs.
  • Participates in all necessary team meetings as needed and scheduled.

Required Education, Experience, Certificates & Licenses:

  • Graduate Degree from Accredited Nurse Practitioner Program.
  • Current Arizona State Nurse Practitioner License.
  • Board Certification in the area specific to position being filled.
  • Any combination of education and/or experience that provides the necessary skills required may be considered.
  • Current CPR certification.
  • Participates in continuing medical education programs to maintain professional competence and skills.
  • Driver’s License and Proof of Insurance may be required if requesting mileage reimbursement.
  • Valid Fingerprint Clearance Card required.

Preferred Education, Experience, Certificates & Licenses:

  • Two years of previous clinical experience preferred.

Required Language Skills:

  • Ability to comprehend and compose effective instructions, correspondence, and communications in English in both oral and written format.
  • Bilingual in English and Spanish preferred, but not required.
  • Ability to effectively present information in one-on-one and small group situations to patients, internal providers and staff and other agency staff working in cooperation with the organization.
  • Ability to communicate technical information to non-technical personnel.

Physical Requirements:

  • Ability to occasionally exert enough force to move objects weighing up to 25 pounds.
  • Ability to continuously remain in a stationary position.
  • Ability to constantly move about inside the workplace to assist patients, operate office or medical equipment, etc.
  • Possesses hand-eye coordination and manual dexterity necessary to constantly operate computer, telephone, and other office machinery.
  • Possesses close visual acuity necessary to accurately record and view information on a computer monitor, handwritten and typed documents.
  • Ability to discern the nature of sounds at a normal spoken volume.
  • Possesses range of body motion and ability to exert enough force to lift and move adult patients.

Other Required Knowledge, Skills, and Abilities:

  • Ability to perform medical math that may include the use of ratio/proportion, dosage calculation, metric and household equivalents, roman numerals, abbreviations, and general math including percentages, etc.
  • Familiar with the involuntary commitment process and can ensure patients are referred to an appropriate level of care.
  • Ability to draw and interpret graphs.
  • Ability to skillfully gather and analyze data.
  • Ability to perform a variety of assignments and make decisions requiring considerable independent judgment.
  • Ability to resolve challenges and work under pressure.
  • Displays high level of critical thinking and problem-solving skills.
  • Knowledge of HIPAA and Corporate Compliance rules and regulations.
  • Knowledge of current principles, methods, and procedures for the delivery of medical evaluation, diagnosis, and treatment within the scope of field of specialty.
  • Knowledge of current, evidence-based preventative primary care within the scope of field of specialty.
  • Knowledge of relevant drugs and non-pharmaceutical patient care aids.
  • Knowledge of health care plans and community health centers.
  • Computer literacy required.
  • Ability to employ motivational techniques to train, mentor, and lead staff.
  • Ability to develop and present educational programs and/or workshops.
  • Ability to prioritize and plan work activities, use time efficiently and develop realistic action plans.

Work Environment & Conditions:

  • Work environment is typical of a health clinic setting with occasional exposure to communicable diseases, bodily fluids, and hazardous chemicals.
  • Work occasionally involves use of needles and other medical equipment with proximity to electrical current, and exposure to moving mechanical parts and some hazardous chemicals.
  • Work requires reliable transportation as position requires occasional travel and extended hours to include early mornings, evenings, and weekends.
  • Work requires ability to be contacted by cell phone during or outside of regular work hours.

Role Title: Full-Time, Internal Medicine Registered Nurse (RN)

Location: Upper West Side, New York, NY

Role Overview:

The Registered Nurse (RN) will be responsible for working with Harley Street Medical’s physicians to provide the highest level of primary medical care to patients. In addition to some of the typical nurse duties outlined below, this role will involve working alongside the physicians to provide highly personalized patient care, facilitating care coordination and helping to scale clinical operations as the practice grows. In providing high touch care alongside the physicians, this candidate should demonstrate the highest level of kindness and empathy in delivering outstanding care to patients. This candidate must be a highly effective communicator and a strong team player. Additionally, this individual should have an entrepreneurial spirit, willing to take an active hand in collaborating across departments and creating unparalleled patient experiences.

Responsibilities and Duties: 

  • In collaboration with the physicians, perform comprehensive physical examinations and perform / order other appropriate diagnostic tests, including drawing patient blood for testing in our lab
  • Administer immunizations and IVs, as necessary, as well as perform EKGs 
  • Professionally maintain treatment areas, medical equipment and other clinical supplies
  • Assist physicians with clinical procedures and document appropriate care rendered in the EMR
  • Keep track of clinical inventory and reorder supplies, as needed
  • Respond promptly and effectively to inquiries from patients regarding their medical care, and assist patients with coordination of services in a professional manner

Qualifications

  • Experienced Registered Nurse (minimum of 4+ years of demonstrated direct clinical experience); Internal Medicine clinical experience is desired
  • Currently licensed to operate in the state of New York
  • Bachelor Degree (preferred)
  • Exceptional interpersonal and communication skills (verbal and written)
  • Strong attention to detail and highly organized
  • Ability to be an effective contributor on a small, tight-knit team
  • Working knowledge of Microsoft Office (preferred)

Astoria Birth Center and Family Medicine is a practice like no other. We are a hybrid team that takes a pragmatic, holistic approach to family care and birth. We are a busy birth center and family practice with exceptionally good statistics. We are seeking either a CNM or a dual-certified Family Nurse Practitioner-Nurse Midwife candidate to round out our birth center and family practice team.

Our ideal candidate will work in conjunction with our existing team, take three clinic days a week with both maternity and family practice clients, and share call time between three providers.

We are passionate about offering exceptional healthcare in a rural health setting and providing a much-needed option to our community that would otherwise not be available.

Applicants ideally have out-of-hospital birth experience (though not strictly required), love working with a high degree of autonomy (read: we do not micromanage) in a beautiful, waterfront location, and are comfortable with engaging with our local community on many levels. Astoria is tight-knit. We help each other, we refer to our local businesses, and we work side-by-side to keep our community awesome. Excellent hospital and government collaboration skills are a must, as we work closely with our local hospital, state legislators, and Medicaid providers to improve patient safety and experience.

We have been and continue to steadily expand.
