Ophelia Empleo-Frazier, MSN, GNP-BC, CDP
About
A graduate of Yale University School of Nursing with a Master’s degree as a Geriatric Nurse Practitioner, Ophelia Empleo-Frazier ’99 is currently a lecturer for the Adult/Gerontology Primary Care Nurse Practitioner (AGPCNP) Program, as well as a clinical preceptor for Nurse Practitioner students in their specialty rotations where she received the Preceptor Appreciation Award in June 2017. She earned a BSN degree from Saint Louis University at Baguio City, Philippines.
She is the owner of Frazier Consulting, LLC which she established in 2014. She serves as a nurse consultant and educator for the Connecticut Older Adult Collaborative for Health 4Ms Project at the Yale School of Medicine’s Geriatric Department since 2015 and is a faculty member in the Weitzman Institute Project ECHO Nursing Home COVID-19 Action Program.
She has been a member of the Sigma Theta Tau – Delta Mu Chapter since 1999 and the Philippine Nurses Association of America (PNAA) where she served as the president of the Connecticut Chapter from 2014 until 2016. She is also on the Board of Directors of the Philippine Nurses Association of America Foundation (PNAAF), the philanthropic arm of the PNAA, as well as the Education Committee Chair of the 2023 6th PNAAF International Collaborative Education Conference. This conference will offer educational programs to local professional nurses and students in collaboration with local nursing schools and nursing organizations in the Philippines. It will also promote and explore innovations in education, practice management, research, and global linkages in nursing.

Research Interests
Ms. Empleo-Frazier’s scholarly inquiry encompasses “Age-Friendly” older adult care focusing on the 4Ms model (matter’s most, medications, mentation, and mobility), maintenance of activities of daily living (ADL) through mindful and functional movement exercises, and caregiving in the older adults.
Clinical Practice
The focus of her clinical practice has been in providing care to geriatric patients in primary care, skilled nursing and assisted living facilities, and in the comfort of their homes. She is a certified dementia practitioner through the National Council of Certified Dementia Practitioners and wound care certified through the National Alliance of Wound Care.
