Debbie Sylvester is a Geriatric Nurse Practitioner and is the Director of Acute & Post-Acute Care for Commonwealth Care Alliance a Massachusetts special needs health plan.
Debbie’s current work is focused on leading an innovative Transitions of Care Program. The mission of the program is to improve the transition of care for members in and out of hospitals and skilled nursing facilities in Massachusetts. The Program seeks to improve member health outcomes, reduce facility readmissions, optimize length of stay, and reduce unnecessary use of high intensity facility-based care. She holds a Masters in Nursing Science from Mass General Institute of Health Professions and a Master in Business Management from Lesley University.
Debbie’s DNP project, focus is to expand on her current work of improving transitions of care for the geriatric population as they move in an out of various sites of inpatient care.
